I understand that Safe Infant Sleep is providing this service using the available public information as of the date listed on the agreement.

 

Should any unknown hazards arise Safe Infant Sleep is not liable, but will notify client of any hazards promptly if they hear of them. 

 

By checking the box in the form you agree to request the service with the knowledge that the information is provided with all the current available data from the AAP, NIH, CPSC and FDA and Safe Infant Sleep is not liable for any information that may come after the fact.
